At first, choose a local newspaper where you can give advertisement regarding to change of your name. You need to include your old name, new name, father’s name and residential address in the advertisement. However, most of the newspapers have a common format for this.

Then you need to get an affidavit duly attested by first class Magistrate/ Notary mentioning your full address, father’s/ husband’s name in affidavit. And if you are working with Central Govt., you can submit deed for change of name in accordance with the Ministry of Home Affairs O.M. No. – 190016/1/87-Estt. Dated 12.03.1987. Those who are Indians but living in abroad, they have to submit deed duly attested by the respective Indian Embassy/Indian High Commission, in original to this Department.

You need to give details on a blank sheet of paper such as (old name, new name, Father’s name, permanent address and the name of employer) and which you must sign in the presence of two witnesses and witnesses also need to sign on the same paper.

Once you completed aforementioned formalities, you need to send all these documents to “Controller of Publication, Department of Publication, of your State or you can either visit the department physically. You need to send documents such as the original advertisement published in the local newspaper, original declaration statement, attested affidavit and 2 passport size photographs & the Printing fee of gazette.
